The applicant shall provide the business tax division with copies of all licenses,
certificates, or similar proofs of licensure relating to the applicant's occupation,
when state regulated. The applicant shall further provide the business tax division
with an affidavit stating the applicant has complied with all federal, state, and
local laws, regulations, or rules and appropriate certificates, whether health certificates
or otherwise, from all appropriate agencies relating to that occupation.
