A sworn list of businesses owned and operated or managed by applicant or in which the applicant has had an interest of any kind during the past five (5) years and the addresses of these businesses is required. Affidavit of experience signed and notatized by a licensed contractor, architect or engineer verifying work history of applicant for the last five (5) years.
